			<h2>Course Locations and Dates</h2>

			<p>Course availability and dates are subject to change.  Cin&eacute;-Med will contact you to confirm a specific date and site.</p>

			<p>Courses are limited to 2 physicians.  Early application is recommended.  Application and credentialing standards are based on individual site and/or state requirements.</p>

			<p>Physicians will receive course confirmation once Cin&eacute;-Med has received approval of temporary privileges from the site, and issuance of state temporary license, where applicable.</p>

			<p>Course participants are responsible for their own airfare, ground transportation, and hotel expenses.</p>
